ISS Service Request Form

Request Title - Provide a short title for the service you are requesting.
Request Justification - List the reasons for submitting this request (e.g., policy change, new business practice, new functionality desired, business need not met, law changed, etc.).
Functional Details - Provide specific details about your request. This is important for ISS to gain full understanding of the request.
Priority - Describe how this work fits in relationship to current ISS requests for this office.
Supervisor Approving Request (name, title, phone extension)

This form is to be used when requesting CPIT ISS services such as new customization development, modifications to existing programs, interface development, system integration, data migration/conversion, new system implementations, technical research or system upgrades.

For all other CPIT ISS system issues such as production system outages, problem resolution or server/database administration services, please submit an on-line ticket at http://ouch.cua.edu. If you have an emergency system problem contact the CPIT Information Center at 202-319-4357 between 8 AM and 9 PM Monday through Friday. To report a system wide emergency during all other hours, contact the Department of Public Safety at 202-319-5111.
